AUTOSCALER® ULTRASONIC SCALER/PROPHYLAXIS
UNIT FOR MODELS AW25C AND AW30C QP273 REV:D.
FOR AUTOSCALER®
UNITS WITHOUT HP-DIAG LIGHT FEATURE.
WARRANTY
South East Instruments™ Corporation (hereafter SEI)
subject to the terms, and conditions hereinbelow
expressly set forth in Paragraphs "1" through "4"
HEREBY WARRANTS that it will repair the AUTOSCALER® Ultrasonic Scaler which proves
defective by reason of improper workmanship and
or materials.
1. All devices sold June 1, 2003 or later will have
a 6 YEAR Parts and Labor warranty on internal
circuit boards, and 2 YEARS Parts and Labor
on all other components of the AUTOSCALER®. The
longest warranty in the industry.
Until May 31, 2003, warranty is
2 YEARS Parts and Labor
coverage on the entire AUTOSCALER® unit
excluding the following components: (Handpiece
assembly, Water system, and Foot switch which
are covered for one (1) year Parts and Labor).
2. ORIGINAL PURCHASER: This warranty is limited to
the original purchaser of the unit. When requesting
warranty service the following information is
needed by SEI: A copy of the enclosed
warranty card must be submitted to SEI
within 2 WEEKS after installation to validate
warranty. A RETAIL sales receipt may be used to
VALIDATE sales/installation date.
3. PROPER DELIVERY: The unit must be shipped, Freight
Prepaid, or delivered to SEI to render
the services provided hereunder in either its
original package, or a similar package
affording an equal degree of protection.
Warranty does not include shipping and handling
back to the customer.
4. UNAUTHORIZED REPAIR, ABUSE, ETC.: The unit must
not have been previously altered, repaired or
serviced by anyone other than a service
facility authorized by SEI. To render
such service, the serial number on the unit
must not have been altered or removed. The unit
must not have been subjected to accident,
misuse, abuse, or operated contrary to the
operating instructions. Handpiece assembly is
not warranted if subjected to abuse from
excessive pulling/stretching, debris and/or
chemical accumulation from water supplies.
Water system is not warranted by failure due to
debris, and/or chemical deposits from water
supplies. Foot switch is not warranted by
failure due to operation or storage in a wet
environment.
EXCEPT TO THE EXTENT PROHIBITED BY APPLICABLE LAW,
NO OTHER WARRANTIES, WHETHER EXPRESS OR IMPLIED,
INCLUDING
WARRANTIES OF MERCHANT ABILITY, AND FITNESS FOR
A PARTICULAR PURPOSE, SHALL APPLY TO THIS
EQUIPMENT. DAMAGES ARE LIMITED STRICTLY TO
REPAIR OR REPLACEMENT OF PARTS. UNDER NO
CIRCUMSTANCES SHALL SEI BE LIABLE FOR
INCIDENTAL OR CONSEQUENTIAL DAMAGES RESULTING
FROM THE USE OF THE EQUIPMENT, AND SEI NEITHER
ASSUMES NOR AUTHORIZES ANY REPRESENTATIVE, OR
OTHER PERSON TO ASSUME FOR IT ANY OBLIGATION
FOR LIABILITY OTHER THAN SUCH AS IS EXPRESSLY
SET FORTH HEREIN.
Extended
Warranty
This (1) one year extended
warranty, for $199.00, adds on an extra year to our
existing warranty of (2) two year parts and labor
while the circuit board remains at (6) years.
Customer is responsible for shipping and handling in
both directions. This warranty is only valid if
purchased within 90 days of original device
purchase. Once customer purchases warranty, they
will receive one free DF10 or DF100 insert. The unit must not have been subjected
to accident, misuse, abuse, or operated contrary to
the operating instructions. Handpiece assembly is
not warranted if subjected to abuse from excessive
pulling/stretching, debris and/or chemical
accumulation from water supplies. Water system is
not warranted by failure due to debris, and/or
chemical deposits from water supplies. Foot switch
is not warranted by failure due to operation
or storage in a wet environment.
Once you
have registered your AUTOSCALER®
and we have received payment, an official
certificate will be sent to you via email or
provided mailing address.
